These benefits are payable under the guidelines set forth in the Trust Medical Plan and Trust Dental Plan.

1. Annual pap and pelvic examination;
   
2. Annual breast examination;
   
3. Annual mammogram;
   
4. Annual prostate examination;
   
5. Well-baby exams for the first three years;
   
6. Immunizations; and
   
7. Dental exams and fluoride treatment allowed twice per calendar year;
