The Active Employee Plan makes the Trust Medical Plan available to you as an option for medical coverage. If you select this option you will receive your prescription drug benefits through Kroger Prescription Plans and your vision benefits from VSP. You have the option of receiving dental benefits through Kaiser Permanente OR through the Trust Dental Plan.
The cost of coverage through the Trust Medical Plan may be different than similar coverage through the other options offered in the Active Employee Plan. See Plan Options & Rates for the current cost of coverage.
Disease Management - Living Well with a Chronic Condition
The Disease Management program is a voluntary program that helps participants with chronic conditions by providing information and support to help them take an active role in their own health. The nurses at Innovative Care Management work on an individual basis with participants, their families, and their doctors to create a custom plan to best fit the participant’s needs. This plan can consist of components such as:
- Educational materials
- Contact with the treating physician
- Specialist referrals
- Medical equipment recommendation
- Home health services
Through these services, the Disease Management program can help a participant take control of their chronic condition, leading to a healthy, full life.
If you or your dependent covered under the Trust Medical Plan has been diagnosed with one of the following diseases and could benefit from this program, contact the Plan Administrator.
- Diabetes
- Respiratory diseases (e.g. asthma and emphysema)
- Non-metastatic cancer
- Heart disease
- Chronic obstruction pulmonary disease
The Plan Administrator will contact Innovative Care Management and one of their nurses will make contact with you. Participation with the Disease Management program is entirely voluntary.
