Economic Dislocation

Within four weeks of the week that you meet the eligibility requirements, you should complete a claim form to receive benefits. You have 15 days after the end of the Plan Year to submit a completed claim form. The Plan Year ends on December 31. For example, if you met the eligibility requirements for economic dislocation benefits in 2004, you must submit the completed claim form by January 15, 2005. The claim form requires you to provide proof of the following:

  • Documentation of involuntary termination of employment;
  • Travel letter, or other proof that you are relocating outside Local 48;
  • A completed IRS Form W-4 if you do not want the standard withholding of married with 2 exemptions.

To withdraw the second half of your Wage Replacement Account, you must submit a claim form and provide proof that:

  • You have signed an IBEW Local's out-of-work list in the new location; or
  • You have obtained work from a contractor signatory to a collective bargaining agreement with another IBEW Local Union in the new location.

Claims not submitted within given time limits will be denied as untimely by the Plan Administrator.
