Supplemental Enemployment
Within four weeks of the week that you meet the eligibility requirements, you should complete a claim form to receive benefits. You have 15 days after the end of the Plan Year to submit a completed claim form. The Plan Year ends on December 31. For example, if you met the eligibility requirements for supplemental unemployment benefits in 2004, you must submit the completed claim form by January 15, 2005. The claim form requires you to provide proof of the following:
  • Documentation from the state unemployment office demonstrating that you are eligible for, or have exhausted, state unemployment benefits;
  • Documentation that you are registered on the Union's out-of-work list; and
  • A completed IRS Form W-4 if you do not want the standard withholding of married with 2 exemptions.

Proof can include documentation from the State Unemployment Office or a copy of an unemployment compensation check that shows the week(s) covered.
