Medicare Supplement/Medicare HMO Plan

Termination of Coverage
For the Retiree
Your coverage under the Medicare Supplement/Medicare HMO Plan will end on the last day of the month in which any of the following events occur:
  1. You fail to make a self-payment within the time limits established by the Board of Trustees;
  2. The Board of Trustees change the eligibility rules and you cease to be in a class of persons eligible for benefits under the Medicare Supplement/Medicare HMO Plan;
  3. The Board of Trustees terminates the Medicare Supplement/Medicare HMO Plan or the Retired Trust Plan;
  4. You fail to remain continuously enrolled in Parts A and B of Medicare;
  5. You become covered under the Active Employee Plan through active employment; or
  6. 6. You pass away.

For Dependents
Your (the retiree) dependents will lose eligibility for coverage under the Medicare Supplement/Medicare HMO Plan on the last day of the month in which any of the following events occur:
  1. The date your spouse or child ceases to meet the definition of a dependent;
  2. The date your coverage under the Medicare Supplement/Medicare HMO Plan ends for any reason other than death; or
  3. The date your dependent fails to make a timely self-payment.

In the event your coverage under the Medicare Supplement/Medicare HMO Plan ends for a retiree because of death, your dependents can continue on the Medicare Supplement/Medicare HMO Plan.

Coverage under the Medicare Supplement/Medicare HMO Plan must be continuous for you and your dependents. If you enroll in the Medicare Supplement/Medicare HMO Plan and subsequently leave the Plan for a reason other than to return to active employment, you will not be allowed to reenroll in the Medicare Supplement/Medicare HMO Plan.
