| Enrollment Procedures |
| Within 90 days before reaching age 65 or otherwise becoming eligible
for Medicare, you should contact the Plan Administrator and select the
Medicare Supplement/Medicare HMO Plan in which you wish to enroll.
You will need to file an application with the Plan Administrator so your exact eligibility status can be determined. Once you have enrolled in a Medicare Supplement/Medicare HMO Plan, you may change your selection during the open enrollment period held annually by the Trust during the month of November for new coverage effective January 1. |
