| Retiree Medical Plan Options |
If you meet the eligibility requirements for the Early Retiree Plan, and you are not eligible for Medicare because of age, you may choose to enroll yourself, your spouse, if under age 65, and eligible dependents for group medical, prescription drug, dental and visions benefits. There are four options available to you: 1. You may choose the Retired Trust Plan's group medical, prescription drug, dental, and vision benefits (using Kroger Prescription Plan to provide prescription benefits and VSP to provide vision benefits). If you reside in the Kaiser service area, you may choose Kaiser dental benefits. 2. If you reside in the Kaiser Permanente service area, you may choose to have your medical, prescription drug, and vision benefits provided by Kaiser and have your dental benefits provided by Kaiser, Willamette Dental or the Retired Trust Plan. 3. If you reside in the Providence service area, you may choose to have your medical and prescription drug benefits provided by Providence. You will have vision and dental benefits provided through the Retired Trust Plan (using VSP to provide vision benefits). If you reside in the Kaiser service area, you may choose Kaiser dental benefits or Willamette Dental. |
