Home Maker Services
This benefit is available if you are covered under the Trust Plan, Kaiser Permanente, or Providence Open Option. This benefit is not available to individuals on a Medicare Supplement/Medicare HMO Plan.

This benefit is available when you are physically unable to perform daily household tasks and no other household member is able to perform these tasks. The out-of-pocket cost for home maker services will be reimbursed by the Trust at 80% of the amount charged, to a maximum of $100 per week. A week commences the first day charges for home maker services
are incurred. The lifetime maximum benefit for these services is $5,000. For example:

To be eligible for this benefit, you must meet the following guidelines:
  1. You must formally apply to the Plan Administrator and demonstrate both cause and need; and
  2. Services cannot be rendered by a family member.
