VSP Vision Benefits
The VSP Vision plan is provided by the Trust to accompany the Trust Medical plan as well as the HMO Oregon and Regence Blue Advantage Plans. If you select the Kaiser plan, Kaiser provides vision benefits with that plan.


What Is Covered and What Is Not Covered
Services Covered

Vision Exam: Includes a refraction test to determine the need for glasses, analysis for binocularity, and testing of the overall health of the eyes and related optic structures. This benefit is available once every 12 months from the last date of service.


Eyewear Covered
  1. Frames and lenses: Benefits for lenses are covered in full and available once every 12 months from the last date of service. Benefits for frames are covered up to $130 and available once every 24 months from the last date of service. Before you select your frames or lenses, check with your VSP network doctor to find out which frames and lenses are fully covered by this Plan.
  2. Contacts: When you choose contacts instead of glasses, your $105 allowance applies to the cost of your contact lens exam (evaluation and fitting) and contacts. The exam is in addition to your exam to ensure proper fit of contacts. This benefit is available once every 12 months from the last date of service and is in lieu of the lenses and frame benefit.

Services NOT Covered
  1. Orthoptics or vision training, and any associated supplemental testing;
  2. Medical or surgical eye treatment; and
  3. Eye exams or corrective eye wear required by an employer as a condition of employment.

Materials NOT Covered
  1. Plano lenses;
  2. Two pairs of glasses instead of bifocals; and
  3. Replacement of lenses or frames furnished under the VSP program that are lost or broken before the 12 or 24 month period is completed.

Additional Costs
  1. Cosmetic contacts over $105;
  2. Oversize lenses;
  3. Blended lenses;
  4. Laminated lenses;
  5. Frames that cost more than the Plan allowance; or
  6. Photochromatic lenses or tinted lenses for dependents.

Special Conditions
If you need an eye exam or new eyeglasses before your 12 or 24 month period is completed because of a medical condition, you should have your doctor submit a prior authorization and a request for payment authorization to VSP. After review and upon the Trust's approval, VSP may authorize services under the Plan.
